Facilitator-Teacher Evaluation Form for LAC Sessions
Evaluation Form (LAC Session) is designed to assess primarily the whole activity. This includes the Facilitator and the andragogy used in the conduct of LAC.
The format below can be modified depending on the need of the activity. It could also serve as platform if you wish to need an Evaluation Tool on other activities.
USAGE
Just take into consideration that the Evaluation Form is not used as criticism on the facilitator. This tool is used to develop personal and professional growth. It does not affect teacher’s performance on the KRA Standards. This tool is used to supplement other approaches to help teacher and teaching development.
