Modified Learner Enrollment and Survey Form (MS Excel Format)

The Department of Education has released the revised version of the Learner's Enrollment and Survey form (LESF) which was distributed last year due to pandemic. The said form was used to collect data from the learners in order to possibly address the current needs and predicaments of our students whilst facing this pandemic. As far as the Department is concerned, the learning opportunities of our youth should not be terminated just because of this crisis.

For this School Year 2021-2022, a Modified LESF has been released in order to cultivate the different data elements last year in a more accurate and efficient manner, pending the release of a new DepEd Order and a summary report for these forms.

The Modified Learner Enrollment and Survey Form is now available for download in the Learners Information System (LIS). The file is available in both English and Filipino which are saved in Microsoft Word format.

The file is formatted on MS Excel 365 with the following settings:

MLSEF Excel Format is set on an A4 bond paper size
Page orientation is set on portrait mode.
Margins are custom with zero (0) inches except left margin at 0.25"
File is tested on Epson L305 printer
