With the release of DepEd Order No. 3 s. 2018, otherwise known as Basic Education Enrolment Policy, the standards with the enrolment procedure has been clearly defined along with the procedures that should be undertaken. Included in the said DepEd Order is the Basic Education Enrolment Form (Annex 2) under pdf file. We have taken the liberty to encode the said form in MS Excel Format and can be downloaded free below. The file can be edited using MS Office just by clicking on the blank cells where the specified data should be indicated.
